Hi - The default subject for the email notifications will include the form name and the order id. Could you please share a screenshot of the page that shows the word "Test" instead?

You have only set up the confirmation emails for this form and it does not have the subject or the message displayed in the screenshot. Could you please check and confirm the "From" address for the email in the screenshot above.
