The first page in the form with the required fields will be set as the checkout page by default. You have added the required fields in the home page. That's why it is set as the checkout page.

Login to Neartail > click on the form to open it > Edit page will be displayed > In the Edit page, click on the 9 dots grid icon on the bottom right corner at the end of a section (see screenshot below) > select the preferred page for the "Configure the checkout page" option and click Save.
