Dear Advisers,
Please fill out this form based on the order placed by the students in your respective advisory classes.
The placement of all the orders must be done by Tuesday, October 31, but the deadline of the payment will be on November 6, 2023. It must be coursed through the accounting office, which will prepare an acknowledgement receipt per class.
The distribution of fully-paid shirts will be done on or before November 10, 2023.
Make sure to take a screenshot everytime you place an order for documentation and verification purposes.
